"BUILDING BRIDGES TO HEALTH"
AN EARLY DETECTION & PREVENTION HEALTH FAIR

Dear Participants:

We are pleased to announce that you, your spouse and your eligible dependent children who are at least 18, have the opportunity to participate in one of five "Building Bridges to Health" fairs being offered Saturday during the months of October and November 2011, at no cost to you. These health fairs are being held at various Union facilities in Aurora, Burr Ridge, Chicago , Merrillville, Indiana and Volo. The Roofers' Unions Welfare Trust Fund is co-sponsoring these health fairs, along with several other Unions and the Midwest Employee Benefit Funds Coalition, of which we are a member.

The program will consist of 36-panel blood test. Men over 50 will receive a PSA blood test and women over 40 will receive a thyroid test. These tests will be conducted at no cost to you. Optional flu shots and bone density testing will also be available at the health fair at no cost to you, along with body mass index testing, dental screenings, access to a pharmacist and an educational session with a nutritionist which will be held at 8:30 a.m. and 10:30 a.m. at each location.

The Roofers' Union Welfare Trust Fund would like to do our part to ensure that you have resources necessary to live a healthy life. This process will give you: 

  • Understand what their numbers mean (i.e. what does it mean if blood pressure is 130/95)
  • Improve less than optimal numbers or maintain good numbers
  • Full lab analysis report with the ability to forward it to your primary care physician.
  • Personal health management support, including health coaching.

 We assure you that this program is completely confidential and is administered by an independent service provider, Interactive Health Solutions (IHS). No one at the Roofers' Unions' Welfare Trust Fund will have access to your personal health information. To get the best results from this initiative, we need high participation and your support. You should register and make an appointment for the blood test by contacting www.interactivehs.com or by calling 1-800-840-6100. A 12-hour fast is required. Remember, for those of you who participate in the blood test, you will be entered into a raffle to win a Wii or Wii Fit Game.

The Midwest Employee Benefit Funds Coalition is pleased to announce "Building Bridges to Health".

This program is to encourage our participants to move toward better health. We have a multi-year approach in mind. In our first year we offered comprehensive blood tests, flu shots and bone density tests. This year we are adding additional screenings including body mass index testing, dental screenings and access to a pharmacist. In addition, we will have a nutritionist doing an educational session at 8:30 a.m. and repeating at 10:30 a.m. at each event.

As a result of this program we want our participants to:

  1. Know their numbers (blood pressure, cholesterol, glucose, etc.)
  2. Understand what their numbers mean (i.e. what does it mean if blood pressure is 130/95)
  3. Improve less than optimal numbers or maintain good numbers

 Our 2011 schedule of health fairs are:

  • October 8 - Aurora, Illinois - Plumbers Local 501
  • October 15 - Merrillville, Indiana - Operating Engineers 150
  • October 22 - Burr Ridge, Illinois - Teamsters 731
  • November 5 - Chicago, Illinois - Operating Engineer Local 399

We are in the process of obtaining a location within Lake County for either October 1st or November 12th.

Participants will be able to go to any location to participate. Coffee and juice as well as pastries and fruit will be available at each location. 

We have selected Interactive Health Solutions (IHS) to conduct 36-panel blood tests. There will be no cost for these tests for all Local #11 Members and includes the blood draw and analysis, a very comprehensive report of the results, access to IHS' telephonic coaching and on-line access to their medical database. In Addition, women over 40 will receive the thyroid test and men over 50 will receive the prostate specific antigen (PSA) test. These tests will be done at no cost for Local #11 Members. Flu shots will be available also at no cost for Local #11 Members and all other procedures are available at no cost to the Fund or the participants.

IHS, in conjunction with the participating Member Funds, will create the communication pieces that will be sent to your participants. IHS will handle the pre-registration the day of the event. Participants will be able to register by phone or on-line. Registration will end at close of business the Wednesday before the event. At the end of the five weeks, IHS will prepare a detailed listing, by Fund, of those who participated. This invoice will be sent to each Fund for payment. A similar detailed listing of those who received a flu shot will also be prepared and sent to the Funds.

Frequently Asked Questions (FAQ)

1. When and Where is this taking place?

Our "Building Bridges to Health" Fairs will take place on Saturday, October 8, 5, and 22 and November 5, 2011. The program will start at 7 a.m. and go until 12 noon. The locations of the health fairs are Aurora, Burr Ridge, Chicago (South Side) and Merrillville, Indiana. Various Unions or Fund Offices have offered us access to their facilities.

Participants can attend any of these events depending on which date or which location is most convenient.

2. What will be offered at the "Building Bridges to Health" fair?

At this event participants can receive a comprehensive 36-panel blood test along with a flu shot, a bone density test, a body mass index test, dental screenings, access to a pharmacist and an educational session with a nutritionist. Refreshments will also be provided as this is a fasting blood test.

3. What is the cost?

There are no costs involved in the program for Local #11 Members.

4. Who can attend/participate in this event? Is there an age limit?

We are recommending that participants older than 18 participate in this event. However, each Fund will determine which participants to invite. Therefore, you decide if you want to include apprentices, actives, retirees, spouses, etc.

5. How will registration be handled?

Interactive Health Solutions (IHS) will handle the registration. we want participants to pre-register so that we can properly staff and supply each location. Participants can register by phone or on-line. Registration will be stopped at the close of business on the Wednesday immediately preceding the event. A list of registrants will be sent to each Fund for review and authorization by that Friday.

6. What about walk-ins?

Walk-ins will occur and can be handled the day of event. It has been some Funds' experience that there will not be a lot of walk-ins and they offset those who register and do not show. Walk-ins will be asked for identification and the name of the Fund that provides their benefits.

7. How will the billing be handled?

Both the company providing the blood tests (IHS) and the company doing the flu shots will prepare a detailed listing of each Fund of the names of the participants that had a blood test or received a flu shot. The billing will be prepared at the end of five (5) events, Each Fund will be responsible for sending payment to these companies.
